To be considered for surgery, you must meet these requirements:
Be between 15 and 50 years old
Have a
BMI
in the range of 40-50, or 35-39 with life threatening
co-morbidities
, and greater than 75 lbs. overweight
Have had no previous weight loss surgery
Failure with nutritionally and medically supervised diets
