What is needed and what to say to your PCP:
Have your primary care physician “medically clear you for surgery”. Dr. Thomas will send a letter and a complete copy of your intake information. He will ask that a complete medical evaluation be done to include a review of cardiac, respiratory, GI systems and hematological disorders and that pertinent medical records be forwarded along with his medical clearance.
This is a routine process between physicians. You may want to take a copy of this message if you see him/her before he receives the letter and you are unclear why you need to see your doctor. You may be asked to sign a release of information to send records and that is normal.
